Change of Contact Information for the Diocesan Health and Welfare Benefits Trust

This change includes all communication, including (a) medical plan enrollments for active and retired clergy and lay employees, (b) disability and life insurance enrollments and questions, and © all copay and mental health rider claims and questions. Parke Rouse will continue to serve the Trust as its Administrator.
Note: Any hand-delivered mail and/or packages can continue to be left for Parke Rouse or the Trust at Church House. Monthly payments should continue to be sent directly to TD Bank at the DHWBT lockbox at P.O. Box 95000-5001, Philadelphia, PA 19195.
As of January 1, 2010, the Patient Partner address and phone number are no longer correct contacts for any Diocesan matters, as Fran Murphy won’t be assisting the Diocese with daily operational tasks in the future. Fran will continue to work with the Commission in other areas.
